WordPress file changes

I am starting to be a little worried about this. It happens more often I would say and it doesn’t seem like some regular updates of files for me.

Could anyone troubleshoot what is happening with my site through these files?

I have two web pages what have been hacked before. I cleaned site and make fresh installation and install wordfence plugin, what seems like it is working.

Both of these pages are running on WordPress 4.9.8.. I have outdated version because if I update on 5.+ my powerbuilder plugin on site will stop working.

Anyway, what wordfence says in scans is that WordPress core file was modified. I assume that this thing got nothing to do with plugins updates since it is in wp-includes and wp-admin folders on web server.
