Those address ranges look good to me, I didn't recognise 198.18.0.0/15, but apparently it is reserved for home routers: http://www.rfc-editor.org/rfc/rfc2544.txt and is a private range too.

Getting the first public IP from the left of the X-FORWARDED-FOR header is what I would expect the algorithm to do.

Looking forward to the next release.
